Service Description
Provides health and support services for people who wish to remain in their homes. Offers the care and support necessary for people to live as independently as they can. Services include:
Assessor Coordinator – assesses health and related social needs, and develops a care plan through case management services on a short term or long term basis depending on needs.
Nursing: Specialized care that assists clients in meeting their medical and personal needs. Nurses may provide:
- Medication management
- Wound care ad prevention of wounds
- Intravenous therapy
- Diabetes education
- Tube feeding
- Ostomy care and catheter care
Home Service: services include dressing, light grooming and bathing, light meal preparation and medication assistance.
Occupational Therapist: assesses equipment needs to ensure home ia accessible and safe.
Physical Therapist: assesses, plans and monitors physical therapy programs.
Social Work: supports clients dealing with anxiety and depression in a community group setting or on an individual basis.
Palliative Care: provides personal and compassionate supportive care to the terminally ill, assisting them to remain in the comfort of their homes during illness. Available for urban and rural. Call 306-766-2674.
Meals on Wheels: a volunteer-based program that delivers hot meals to individuals who are unable to make meals by themselves; the number of meals per week is based on the patient’s needs and availability in the community.
Frozen Meal Service: meals, ordered through REACH (306-347-3224) are delivered by courier and can be heated in a microwave or regular oven.

Qualification factors:
-You and the Home Care service provider in your area agree that you need a health and/or support service that can be provided in your home.
-The program has the resources to meet your needs.
