Service Description
Provides mobile primary healthcare clinics staffed by a multidisciplinary Saskatchewan Health Authority team that includes a Nurse Practitioner, a Licensed Practical Nurse, and a Mental Health Assessor Coordinator who can provide assessments and connect people to needed resources. Core services may include:
- Primary health care services
- Take Home Naloxone kits and training
- Opioid Agonist Therapy/withdrawal management
- Mental health and addiction assessments and counselling/referrals
- Sexually Transmitted and Blood-Borne Infection testing and treatment
- Reproductive health care, wound care, stitches
- Vaccinations
- Basic health assessments (physicals)
- Health education
- Referrals to other services and care providers including housing and social services, addictions and mental health treatment and services, and specialist medical care
- Connection to culturally relevant Indigenous Services (traditional medicine, connection to spiritual services and Elders
- Chronic disease monitoring
Eligibility
Individuals facing barriers to accessing care in more traditional settings.
