Service Description
Assists patients with medical concerns to make positive lifestyle changes. Provides one-on-one and group sessions on various topics including: weight management, exercise motivation, diabetes management, fall prevention.
Eligibility
Common referrals include (but are not limited to) individuals with diabetes, heart disease, osteoporosis, depression, anxiety, weight management, cholestorol, eating disorders, blood pressure, stroke, MS, post polio, or fibromyalgia.
Application Process
Must get a referral from a physician or allied health care professional including nutritionists, nurse practitioners and counsellors. Contact for details.
