Service Description
Provides home visits to women and their families on a bi-weekly basis and on an as needed basis. Home visits include:
- Delivering milk coupons
 - Assistance such as transportation to health care
 - Advocacy and Intervention
 - Information/education regarding pregnancy, infant care, breast feeding, parenting, etc.
 - Referrals and support to resolve family crisis related to addictions, violence, homelessness, criminal justice involvement, etc.
 - Social support, problem solving, and encouragement.
 
Eligibility
Pregnant women and women within first 3 months after childbirth residing in the city of Prince Albert.
Also, pregnant women or women within the first 6 months after childbirth residing within rural areas of Melfort, Nipawin, and Tisdale.
Application Process
Self-referral or agency referred. Call for details.
