Service Description

Provides residential care in hospitals, health centres, special care homes, and affiliate health centres in the Health Region for heavy-needs clients.

Placement into long-term care is based upon a rating system that identifies applicants with the greatest need and least support as priorities for placement. Every effort is made to match the special needs of each client with the long-term care facility, services, location, and environment.

Application Process

The single point of entry to apply to long-term care is through the local Client Care Coordinator who provides guidance to the individual and their family about the application process. The Regional Coordinating Committee then determines which residents have the highest risk and/or the greatest need for long-term care services.
