Service Description
Planned program of activities designed to enhance the quality of life for residents living in the community including social, recreational, therapeutic, intellectual, and spiritual activities. Operates on the premise that support services will assist older adults and individuals suffering from varying degrees of dementia to remain in the community as long as possible. Provides:
- respite
- education and support to participants, family members, and caregivers.
Transportation: Can be arranged for a fee.
Eligibility
Participants must be ambulatory or transfer from a wheelchair independently. Due to staffing levels, individuals who present severe aggressive behavior or who require extensive 1:1 attention cannot be accepted.
Application Process
All participants access the Community Day Program through the Saskatchewan Health Authority's Client Patient Access Services (CPAS). A CPAS coordinator works with individuals and families to provide information about the Day Program. They will identify specific needs, determine eligibility, and facilitate access to the appropriate Community Day Program. Individuals interested in attending the Community Day Program should contact CPAS.
Fees
$9.25 per day
$8 for arranged transportation ($4 per trip).
