Service Description

Palliative Care Coordinators meet with people at home, in hospital or at Regina Wascana Grace Hospice to assess needs, explain the services of the palliative care team, and make a plan of care with the client.

  • Provides management of pain and symptoms, ongoing assessment, personal care, contact between client and doctor, and support to clients and families in their own home.
  • The home care nursing service is available on a 24/7 hour basis and the nurse is available to pronounce death when death occurs at home. Home health aides give personal care and support to the client, respite to care providers, and assist in homemaking and meal preparation.
