Service Description
Personalized service that enables older adults and people with disabilities to remain as independent as they choose. Brings assistance to clients homes, assisted living environments, hospitals or other locations such as long term care facilities.
Eligibility
Bridging coordinator visits each client for a complete assessment. Call to setup an appointment.

Fees based on structure below and may be subject to change due to specialized care needs etc.
3 - 99 = $28.50 per hour
100 - 199 = $27.50 per hour
200+ = $$26.50 per hour
12 Caring Hour Visits = $342
24 Caring Hour Visits = $680 (5% discount)
48 Caring Hour Visits = $1230 (10% discount)
