Service Description
Works to move people who are chronically and episodically homeless as rapidly as possible from the street or emergency shelters to permanent housing with supports that vary according to participant needs.
Supports are provided by a case management team, including: drop-in support, filling out forms, phone and fax access, giveaway items such as clothing, tenant/landlord relations, and referrals to other community resources.
Eligibility
Individuals who are chronically and episodically homeless.
