Service Description
Provides support to clients who are experiencing severe and/or persistent mental illness. Provides case management, treatment and coordination of services including clinical and administrative functions, assessment, counselling, planning, linkage, advocacy and monitoring.
Eligibility
People who are experiencing severe and/or persistent mental illness.
Application Process
Referrals are made by clients treating psychiatrist.
