Service Description
Patient-focused end-of-life care option for eligible Saskatchewan residents
Eligibility
18 years old and have decision making capacity
MAID assessor evaluates eligibility per legislative requirements, patient must meet all criteria
Call MAID or HealthLine 8-1-1 for more information
Application Process
Call, MAID team member will complete initial intake
Application has several steps, visit website for details
Note: MAID team needs to speak with the patient directly to confirm their interest in MAID
