Service Description
Provides home and community based services to families with children who are birth to school age, and are developmentally delayed or “at risk” for delay. Services help identify and implement realistic goals to enhance child development, support families and enhance community connections.
Eligibility
Birth to school-age children who are developmentally delayed or at risk for developmental delays and their families.
Application Process
Families, professionals or other agencies can refer to this program. Referrals can be done over the phone, by mail or fax.
