Families or individuals in need of assistance.
Application Process
Call to make an appointment. All appointments must be pre-booked in advance - in person or by phone.
Documents Required
Photo ID (some locations require Health Card Number), Proof of Address, and Proof of Income.

Prebooked appointments are available once every 30 days on Tuesdays and Thursdays between 1:00 pm - 4:00 pm. After hours pick ups can be arranged. Call for more information.
