Supported Independent Living Program

Provides 14 one-bedroom fully furnished apartments where individuals continue to have access to 24-hour support as needed, but are encouraged to utilize skills developed in the Life Skills Program. Individuals are provided with scheduled one-on-one appointments to assist in areas of need, depending on skills and abilities. Individuals are responsible for maintaining their apartment, attending Day Pramming, scheduling appointments, and using skills learned through the Life Skills Program. Staff work with individuals to establish a reasonable savings plan and the funds to successfully transition into the community independently. Staff assist clients in securing affordable housing and secure items and furniture needed for living independently. Services include:

• development of individual Wellness Plan,
• communication of client needs and collaboration with psychiatric nurses and psychiatrists,
• collaboration with client families and outside supports,
• guidance with personal health and hygiene practices,
• medication/blister pack monitoring, 
• guidance with budgeting and finances, 
• healthy coping strategies education,
• food safety and nutrition planning, 
• social and communication skill development,
• 24-hour crisis intervention support,
• assistance establishing health routines, 
• assistance establishing a positive social network and provide healthy relationship education,
• assistance with scheduling appointments and providing transportation to essential medical appointments,
• advocacy.
