Service Description

Professional and non-professional staff visit people with the greatest need to provide services that are supportive, rehabilitative and palliative; promoting independence, maintaining dignity, and enhancing quality of life.

  • Care provided is to serve as a complement to the support provided by family, friends, neighbours, and others in the community.

Professional services include: nursing, physiotherapy, occupational therapy, social work, nutrition counselling, education, rehabilitation, and palliative care.

Support services include: personal care, in-home respite, meal preparation, home management, meals on wheels, and transportation.

Application Process

Contact Client Patient Access Services (306-655-4346).
Family, friends, or health professionals may also call to request services on an individual's behalf.

Professional services: no direct charge for Saskatchewan residents with a valid Saskatchewan Health Card.
