Service Description
Provides support for adults with significant and enduring mental illness who are considered to be hard to serve and difficult to engage. (i.e. substance disorder, challenging behaviours, homelessness, etc.)
Individuals receive in-person, in-house support. Goals of the program include medical stability, long-term housing, financial stability, diversion from emergency department, diversion from criminal justice system and connection to supports.
Eligibility
Individuals experiencing serial crisis and chronic crisis due to a lack of connection at all or in a meaningful way, including individuals who may have behaviour, mental illness, and non-compliance issues as well as legal, addictions, and special needs such as FASD or ABI.
Application Process
Referrals come from the families, mental health, medical, psychiatrists, social service and legal professionals, lawyers and social workers.
