Service Description
Provides client-tailored support services for individuals, families, caregivers that require various supports to have needs met. Services include:
- Support in accessing community resources
- Information and Advocacy
- Planning to help you achieve your goals
- Collaboration with school teams
- Youth and adult transition planning
- Brief counselling
- Assistance navigating government systems
- Help in accessing funding to meet unmet needs
- Support in obtaining caregivers
- Training opportunities for caregivers, families, and the community
- Monthly social programs
- Summer Recreation and Respite Program.
Eligibility
Individuals, families, and caregivers that require various supports to have needs met.
Application Process
An individual or family member can fill out a referral and consent form available at the CISS office, or referrals can be made through a community agency, school or service provider with permission by the family.
