Service Description
Provides support to community members struggling to find and maintain housing. Activities include:
Weekly Housing List: Develops and publishes a comprehensive housing list.
Housing Assistance: Assists clients to secure suitable housing.
Move-In Support: Provides practical assistance to help transition into new homes smoothly.
Home Essential Items: Aids in locating and obtaining necessary items including bedding, dishes, and other essential household items.
Advocacy for Clients: Advocates on behalf of clients, helping navigate housing and social service systems.
Emergency Hampers: Provides emergency hampers to clients facing immediate food insecurity.
Wellness Checks: Conducts wellness checks to ensure clients’ health and safety.
Follow-Up Support: Offers ongoing support to clients to help maintain housing stability.
