Service Description
Provides a long-term peer home for individuals with cognitive disabilities. Each resident is provided with their own bedroom, and can access shared laundry, shared showers, and shared kitchen facilities. Staff are available 24 hours a day to provide care and ensure that residents have support and transportation for daily activities, such as appointments, shopping and recreational activities.
Eligibility
Individuals with cognitive disabilities who require more personal support.
Application Process
Referrals are only through the Oakdale Program or directly from Community Living Service Delivery (CLSD).
