Service Description
Provides 24 hour care and support for adults with mental and/or physical disabilities in two residential group homes.
Eligibility
Adults with mental or physical disabilities.
Application Process
Application forms are obtained by contacting the agency and must be approved by Social Services and the agency before admittance to the program.
