Service Description

Provides home visits to women and their families on a bi-weekly basis and on an as needed basis. Home visits include:

  • Delivering milk coupons
  • Assistance such as transportation to health care
  • Advocacy and Intervention
  • Information/education regarding pregnancy, infant care, breast feeding, parenting, etc.
  • Referrals and support to resolve family crisis related to addictions, violence, homelessness, criminal justice involvement, etc.
  • Social support, problem solving, and encouragement.


Pregnant women and women within first 3 months after childbirth residing in the city of Prince Albert. Also, pregnant women or women within the first 6 months after childbirth residing within rural areas of Melfort, Nipawin, and Tisdale.
