Service Description
Provides home based services and supports to families with children who are birth to school age, who are developmentally delayed, at risk for delay, or have a diagnosed delay, to identify and implement developmental goals for their children and family.
Eligibility
Children aged birth to school age who are developmentally delayed, at risk of developmental delays or have a diagnosed delay and their families.
Application Process
Families can call the program directly to self-refer. A professional referral is not necessary to access this program but any referrals do require parental permission.
