Service Description
Together with the Department of National Defence (DND), provides headstones, markers or memorials for eligible Canadian Forces members.

Eligible Canadian Forces members.
Application Process
Call the Department of National Defence, 1-800-883-6094 to apply. A requisition from the DND is required to assist with the placement of a DND military-style marker. Upon receipt of this requisition, the Last Post Fund will contact the next-of-kin to offer assistance.
