Service Description
A standardized intake and assessment process for individuals experiencing or at risk of homelessness under one umbrella. Agencies share information in real time and adopt uniform prioritization policies and referral processes. Connects individuals and families to the right housing and other support services as efficiently as possible, based on their preference, level of need, and housing availability.

Individuals and families experiencing homelessness.
