Service Description
Provides specialized support in the areas of: transitioning home from the hospital or detox, follow-up support as requested by psychiatry, crisis intervention, and support from the Regina City Police. Team is made up of a variety of health care disciplines, including: psychiatry, nursing, social work, police, and support staff. Provides short term intensive support, while connecting individuals to longer term services or until stabilized. Some services may include but are not limited to:
- Monitoring of mental health and addiction symptoms
- Health teaching in areas such as medication, understanding addiction and resources
- Crisis intervention and support
- Help planning appointments and follow up
- Liaise between psychiatry and client or support network
- Establish connections with community resources
- Assistance planning for residential and financial needs
- Assistance with medication concerns
- Brief intervention with treatment and safety planning
Eligibility
Individuals, 18 years and older.
