Service Description
Provides support services and case management for children and adults of all ages with varying abilities. Services include:
- Information and Advocacy
- Navigating government systems and programs
- Accessing community resources
- Education and Training
- Community Awareness
- Person-Centered Planning
- Goal setting, planning, counselling
- Youth and adult transition planning
- Respite and mentorship support and more
- Inclusive Social Programming
- Dances
- Youth and adult social programs
- Adult activity and drop-in programming
- Summer recreation and respite programming.
Eligibility
Individuals, families, and caregivers that require various supports to have needs met.
Application Process
An individual or family member can fill out a referral and consent form available at the CISS office, or referrals can be made through a community agency, school or service provider with permission by the family.
