Service Description

Promotes the movement from high school to post secondary schooling or employment.
Provides a coordinated set of activities for secondary students with barriers to employment to ensure an individualized student-centered plan.
The goal of transition is to ensure that students with barriers are prepared for employment, further education, and community living when they leave school, and to plan a successful transition together.


Students enrolled in high school with a barrier looking for help to transition from school into the world of work, or to further their education.
