Service Description

Provides services to help people who need acute, palliative, and supportive care to remain independent at home. Services, based on assessed need, include:

  • Case management and assessment
  • Nursing
  • Personal care
  • Home management
  • Meals
  • Respite
  • Palliative – provides active and compassionate therapies intended to comfort and support individuals and families when the cure of the illness or prolongation of life is no longer the aim
  • minor home maintenance.

Volunteer services vary in each community but may include visiting, meals on wheels, and/or transportation to medical appointments.

Eligibility

Persons who need acute, palliative and supportive care to remain independent at home.

Fees

Fees are established based on income.
