Service Description
Provides home care services to help individuals remain independent in their homes an communities. Includes:
- Home Care – individualized plans developed by client and assessor coordinator; includes meals on wheels as needed (extra charge)
- Wellness Clinic – based on need assessed by a nurse; includes client health history review, basic health assessment, foot care, health counselling, and medication review/teaching as needed
- Adult Day Programs – available at various special care homes (long term care homes) to provide another option to receive personal care, socialization, and recreation for clients who need additional support; includes a meal, a bath, and activities.
Eligibility
Individuals who require assistance to remain independent in their homes.
