Personalized service that enables older adults and people with disabilities to remain as independent as they choose. Brings assistance to clients homes, assisted living environments, hospitals or other locations such as long term care facilities.
Services include but are not limited to:
- Home Cleaning and organizing
- Meal preparation and planning
- Personal, specialized, nursing, palliative and respite care
- Escort to medical and professional appointments, social and recreational outings, family events
- Errands assistance
Eligibility
Bridging coordinator visits each client for a complete assessment. Call to setup an appointment.
