Provides outreach services to families with school-age children.
- Using a wraparound approach, workers provide family-centred services and assist families in meeting their needs through connections with community and informal supports.
- Run in partnership between Five Hills Health Region, Ministry of Social Services, Holy Trinity Catholic School Division, and Prairie South School Divisions.
Eligibility
Families with school-age children who experience barriers to accessing traditional community services.
