Provides services for 18 individuals and families experiencing intermittent, recurring or chronic crisis associated with acquired brain injury. Includes:

  • Assessment
  • Service coordination
  • Crisis/intervention/management addressing clients complex needs
  • Assertive outreach services
  • Skill development and client stabilization
  • Intake screening and caseload availability.

Individuals and families experiencing intermittent, recurring or chronic crises associated with acquired brain injury.
