Provides services for 18 individuals and families experiencing intermittent, recurring or chronic crisis associated with acquired brain injury. Includes:
- Assessment
- Service coordination
- Crisis/intervention/management addressing clients complex needs
- Assertive outreach services
- Skill development and client stabilization
- Intake screening and caseload availability.
