Service Description
Operates 11 Community Homes which provide person-centred support for individuals living with intellectual disabilities. 24/7 support is provided by Disability Support Professionals.
Eligibility
Adults with intellectual disabilities who meet the mandate of the Ministry of Social Services - Community Living Service Delivery.
Application Process
Applications are submitted through the individual's Community Services Worker with the Ministry of Social Services - Community Living Service Delivery.
Documents Required
Must include a Saskatchewan Association of Rehabilitation Centres Application and a recent Daily Living Support Assessment cover sheet.
