Service Description
Provides non-resident members access to special needs equipment (wheelchairs, therapeutic beds and mattresses prescribed by a family physician, etc.) when all other resources have been exhausted.
The equipment remains the property of the Cowessess First Nation and is to be returned to the band office when no longer required.
Eligibility
Registered Band Members living in Regina who require the use of special needs equipment not covered by other health benefits.
Application Process
- Application form and the required documentation for program
- Applicants may apply for reimbursement for items under MAP (Extended Care) upon submission of original receipts
- Applications for assistance under the MAP (Extended Care) are subject to a fiscal year limit of two-hundred and fifty dollars ($250.00)
- If travel assistance is approved, the applicant must provide a signed Medical Transportation form
Documents Required
- Supporting documentation provided by a medical practitioner or specialist demonstrating the Band Member needs.
5.2 Assistance Available:
All necessary documentation shall be provided by the Band Member from a medical practitioner
doctor or specialist. The payment will be issued directly to the service provider
